Evaluation Letter Upload

Neha Patel has requested a letter of recommendation from you as a part of their participation in the Pre-Health Professions Advisory Committee.

On behalf of Loyola University Chicago's Pre-Health Professions Advisory Committee, thank you for taking the time to provide us with a letter of recommendation for our student. We know your time is valuable and to make the most of your experience with whom you are recommending, we are providing you with some comments and guidelines to best communicate some of the information you are able to share.

The American Association of Medical Colleges (AAMC) recommends:

• You please provide your evaluation on letterhead stationery.
• You use the appropriate salutation: "Dear Admissions Committee" or "To whom it may concern".
• You briefly explain your relationship with the applicant. (i.e., how long and in what capacity have you known the applicant)
• You focus on the applicant rather than details about the lab, course, assignment, job or institution.
• You focus on behaviors directly observed when describing an applicant's suitability for a health profession program.
• Additional comparison information is helpful: clarifying comparison groups. (i.e., student in a class, student in your department and/or co-worker with rationale for your comparison)

Additional recommendations to include in your letter:

• Applicant's academic performance and potential. (Thinking, reasoning and science competencies, intellectual curiosity, overall intellectual ability)
• Applicant's maturity, integrity, motivation and other important qualities for a career in the health professions.

Please keep in mind that your letter should not include some information.

• Please DO NOT include the applicant's private information such as social security number.
• Please DO NOT indicate a specific school in your letter.
• Please DO NOT return this form or the letter of recommendation to the candidate.

The AAMC suggests that the competencies listed below can be very helpful to admissions committees when considering letters of recommendation. It is NOT expected that any one letter writer provide information on every characteristic listed below. Statement of Applicant: I understand that a copy of the evaluation attached to this statement will be forwarded, at my request, to health-related institutions or agencies that I explicitly designate. I further understand that, if I waive right of access to this letter, I may not read it, and certify that I will not seek to do so, now or in the future. I also understand that I must select all evaluations (including the Committee letter) to be either all confidential or all non-confidential.

This Family Educational Rights and Privacy Act of 1974, as amended (P.L. 93-380), allows a candidate for admission to waive his/her right of access to confidential letters or statements written on his/her behalf if the evaluation is used solely for the purposes of admission and if the candidate, upon request, is notified of the names of all persons making such evaluations on his/her behalf. Under the legislation, candidates have the option of signing a waiver which is effective insofar as the evaluation is used solely for the purpose of admission.

The student has waived their right to access this letter making your letter submission a confidential letter and the student has submitted their electronic signature as confirmation of this choice.
