• Tax ID number
• Banking Information
• IRS Form W-8BEN/W-8BEN-E/W-8IMY/W-8EXP if FOREIGN VENDOR (Individual or Entity)
• NAICS Certification, if applicable

Note: If your Federal Tax Classification is Foreign individual, please have your completed form W-8BEN ready to attach. If your Federal Tax Classification is Foreign Entity, please have your completed Form W-8 ready to attach.

If you are a foreign individual present on a visa or visa waiver receiving a payment for services provided in the United States (i.e. Lecturer/Visitor/Consultant), you will also receive an email from the secure Glacier Online Tax Compliance System, (support@onlline-tax.net )asking you to create a profile and provide your immigration and tax information in order to assess your income tax withholding obligations.
